Specialist Decision Guide

Which type of spine doctor do I need?

Orthopedic spine surgeon, neurosurgeon, PM&R physiatrist, or pain management specialist — their scopes overlap but are not identical. This guide gives you a condition-aware recommendation.

If you have loss of bladder/bowel control, saddle anesthesia, or bilateral leg weakness — go to the emergency department immediately. Do not use this guide. Red flags →

Step 1 — What's your situation?

Quick rule of thumb

  • New pain without red flags: start with primary care or PM&R.
  • Progressive neurologic symptoms (weakness, gait instability): start with a spine surgeon.
  • Chronic pain not responding to PT: pain management specialist.
  • Considering surgery: a second surgical opinion is always appropriate.
